Why records matter

Accurate records help another clinician understand what was diagnosed, what was done and which materials or components were used. They are particularly important for implants and complex full-arch treatment.

Your take-home checklist

Request the final treatment plan, itemised invoice, pre- and post-treatment radiographs, scan report where relevant, implant brand and component sizes, graft material details, laboratory prescription or restoration material, prescriptions, consent forms and maintenance instructions. Ask for records in a format you can open and store securely.

Keep contact and complication information

Save the clinic’s clinical contact, out-of-hours pathway and written warranty or insurance terms. Understand what is covered, exclusions, required maintenance and who pays for travel if a review is needed.

Share records responsibly

Provide copies to your regular dentist if they agree to support routine care. Keep the originals. If you experience a problem, clear records can reduce delays, although a new clinician will still need to examine you before advising.
